2page 6, line 6, and substitute: "person directed by the attending physician, shall
3provide the patient with written information about the resuscitation procedures that
4the patient has chosen to forego and the methods by which the patient may revoke
5the do-not-resuscitate order.

(b) After providing the information under par. (a), the attending physician, or
7the person directed by the attending physician, shall affix to the wrist of the patient
8a do-not-resuscitate bracelet and document in the patient's medical record the
9medical condition that qualifies the patient for the do-not-resuscitate order.".
