AB658-ASA1-AA1,1,1 At the locations indicated, amend the substitute amendment as follows:

AB658-ASA1-AA1,1,2 1. Page 3, line 15: delete lines 15 to 18 and substitute:

AB658-ASA1-AA1,1,4 " Section 12b. 154.11 (4) of the statutes is renumbered 154.11 (4) (intro.) and amended to read:

AB658-ASA1-AA1,1,6 154.11 (4) Other rights. (intro.) This chapter subchapter does not impair or supersede any of the following:

AB658-ASA1-AA1,1,8 (a) A person's legal right to withhold or withdraw life-sustaining procedures or feeding tubes.".

AB658-ASA1-AA1,1,9 2. Page 3, line 19: before that line insert:

AB658-ASA1-AA1,1,10 " Section 12c. 154.11 (4) (b) of the statutes is created to read:

AB658-ASA1-AA1,1,12 154.11 (4) (b) The right of any person who does not have a declaration in effect to receive life-sustaining procedures or feeding tubes.".

AB658-ASA1-AA1,2,5 4. Page 5, line 24: delete the material beginning with that line and ending with page 6, line 6, and substitute: "person directed by the attending physician, shall provide the patient with written information about the resuscitation procedures that the patient has chosen to forego and the methods by which the patient may revoke the do-not-resuscitate order.

AB658-ASA1-AA1,2,9 (b) After providing the information under par. (a), the attending physician, or the person directed by the attending physician, shall affix to the wrist of the patient a do-not-resuscitate bracelet and document in the patient's medical record the medical condition that qualifies the patient for the do-not-resuscitate order.".

AB658-ASA1-AA1,2,13 6. Page 7, line 15: delete "Withholding" and substitute: "Under the directive of a do-not-resuscitate order, withholding".

AB658-ASA1-AA1,2,15 7. Page 7, line 17: delete "a revocation" and substitute: "the revocation of a do-not-resuscitate order".

AB658-ASA1-AA1,2,16 8. Page 8, line 11: delete "person's" and substitute "of the following:".

AB658-ASA1-AA1,2,17 9. Page 8, line 12: delete that line and substitute:
