Page 4, line 22: after "plan." insert "This paragraph applies to a health maintenance organization, as defined in s. 609.01 (2), or a preferred provider plan, as defined in s. 609.01 (4), only with respect to appropriate and necessary immunizations provided by selected providers, as defined in s. 609.01 (6), of the health maintenance organization or preferred provider plan.".
